Is it physically hard to get on and off trains?

Would it be difficult for an elderly person to on and off Mountain trains? I am concerned about high

step-up and stepping down from higher than normal step.

If your mobility is limited, some of them are indeed a bit of a challenge to get on and off, especially the vintage ones, but there is usually a rail to hang on to, which helps. Staff and fellow passengers are usually quite helpful in assisting, too.

To make things as easy as possible, choose sturdy footwear and keep both hands free when getting on and off (e.g. by having all your things in a backpack).
