Filling out front of Bernese Oberland Pass

1. When filling out the Bernese Oberland Pass on the front I'm supposed to put my name, but for "Land" do I put my country of birth?

2. For Pass/ED-NR do I put my passport number?

This may be simple to some of you but I don't want to put something wrong and then have an issue upon my arrival in Switzerland.

2. Yes, correct.

So I assume they didn't include instructions? Did you buy it through the Swiss Railways?

I bought these passes through Switzerland Travel Centre in Zurich.
